Application for 2002 Fairlington Mews Recreation Pass

To obtain your 2002 Recreation Pass, please complete this form and drop it by, or mail it to: Ann Timmons and David Biette, Recreation Committee Co-chairs, 4638 South 34th Street (Court 9), Arlington, VA 22206. Recreation passes will be available for pick-up at the pool Memorial Day weekend upon presentation of a piece of identification with a photo. Please get your application to Ann and David by May 11, 2002 in order to receive your pass Memorial Day weekend. The code for the 36th Street tennis courts is on the back of the Recreation Pass.

We will accept applications for recreation passes at the pool but won’t be able to issue passes on the spot.

Co-Owners are entitled to one Recreation Pass for each person at least 12 years of age permanently residing in the unit. Non-resident Co-Owners who desire a Recreation Pass should attach an explanation to this application.

Tenants of absentee Co-Owners may obtain a Recreation Pass for themselves and each member of their immediate families, age 12 or over, who reside with them. The application must contain the name, mailing address, and phone number of the Co-Owner (landlord) of the unit or that of the Co-Owner’s representative (such as a real estate management company).

Children under the age of 12 do not need their own cards since they—with one exception—must be accompanied to the facilities under adult supervision. Children between the age of 8 and 12 can come to the pool alone if they have passed a swimming test with the lifeguard and a signed parental consent form is on record at the pool.

Should an individual’s pool or tennis privileges be suspended for any reason, the Board may require the surrender of the Recreation Pass.

Children of Mews residents age 12 or over will be issued their own pass; children age 8 to 11 will receive a pass upon passing a swimming test approved by the lifeguards and written permission from parents.

Passes must be presented for use of the pool and tennis court. For your protection and convenience, no one will be allowed to use any recreational facility after May 25, 2002 without the new 2002 Recreation Pass.
