NOTE 1: You can fill out this form, save it to your computer and email it to JEglin@cmc-management.com, or you can print out the blank form,
complete it and mail it to Jason Eglin at

NOTE 2: This form permits "digital signatures" - If you do not want to bother with this process, just complete the form down to the signature line, print out the form, sign your name at the bottom under "Signed", date the form, and mail it to CMC.

After completing the form down to the signature line, at the bottom of the form Click on "Signature" line to bring up the "Sign Document" box.

Click on "A new digital ID I want to create now"

Click on "Next"

Click on "Next" again

Type your name as you want it signed on the "Name (please print) line"

Add your email address

Click on "Next"

Enter and confirm a password of your choice

Click on "Finish"

Enter Password and Click on "Sign"

Date the form, click on "File" in your browser (e.g., Internet Explorer) and then click on "Save As"

Name your document in the directory of your choice on your computer
At this point, you can email your saved form to JEglin@cmc-management.com.
